Allergies are unique to each person. Our goal was to create a simple allergy form fit for all allergies. Patients would complete this form prior to their visit to maximize the time they spent with a provider.
Creating inclusivity in the patient intake process | 2023
My team and I won design of the year with a form that established trust by allowing patients to communicate how they wanted to be addressed prior to a visit as well as select any concerns they had.
